Please download, print and complete the form to enrol your child/children to Shishukunj Leicester. Once completed, please hand-in your form directly to one of the group leaders at 10:45am on Sunday to complete the enrolment.

Prayer Book Form: Download form

Please download, print and complete the form to request a Shishukunj prayer book (3 weeks after initial enrolment). Once completed, please hand-in your form and the money directly to one of the group leaders at 10:45am on Sunday to gain a new prayer book. Please note this form can also be used to request a new or damaged prayer book.
